Pediatric Skilled Nurse (RN/LPN) – GAPP

Program: Georgia Pediatric Program (GAPP)
Schedule: PRN, Part-Time and Case-Based Opportunities
Flowers Home Care LLC is seeking compassionate, dependable Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide one-on-one skilled nursing care to pediatric clients through the Georgia Pediatric Program (GAPP).
Our pediatric clients may have complex medical and developmental needs and require ongoing skilled nursing services in the comfort and familiarity of their homes.
We are looking for nurses who understand that pediatric home care is more than completing clinical tasks. Our nurses become an important part of the child's care team and work closely with families, physicians, and Flowers Home Care's clinical leadership to promote safety, stability and quality of life.
Skilled Nursing Responsibilities
Responsibilities will vary based on the child's physician orders, plan of care, authorization, and individual medical needs and may include:
• Perform skilled nursing assessments and ongoing monitoring
• Administer medications and treatments as ordered
• Monitor seizure activity and follow established seizure protocols
• Provide tracheostomy care and suctioning when ordered
• Provide respiratory treatments and nebulizer care
• Monitor respiratory status and oxygen needs
• Provide enteral/feeding-tube related nursing care when applicable
• Monitor nutrition, hydration and elimination
• Provide skin integrity and wound-related care as ordered
• Monitor vital signs and changes in condition
• Assist with medically necessary activities of daily living within the nursing plan of care
• Follow emergency and physician-ordered protocols
• Recognize and promptly report changes in the child's condition
• Complete accurate, timely skilled nursing documentation for each shift
• Communicate effectively with parents/guardians and the clinical team
• Follow the child's individualized plan of care and physician orders
• Participate in supervisory visits, care-plan updates and required training
• Maintain patient confidentiality and HIPAA compliance
• Follow Flowers Home Care policies and applicable GAPP/Georgia Medicaid requirements

Minimum Qualifications
Applicants must have:
• Current, active Georgia RN or LPN license in good standing
• Current BLS/CPR certification appropriate for the position
• Ability to successfully complete required background screening and employment requirements
• Reliable transportation
• Ability to follow physician orders and individualized plans of care
• Strong clinical documentation and communication skills
• Ability to recognize and appropriately respond to changes in a pediatric client's condition
Pediatric, private-duty nursing, GAPP, medically fragile pediatric, tracheostomy, seizure, respiratory or feeding-tube experience is strongly preferred.
Training and competency verification will be required for specialized clinical tasks based on the assigned child's needs.
